摘要:

威斯康星米勒菌(Moellerella wisconsensisM. wisconsensis)是一种新发现的潜在人畜共患致病菌,偶尔从人和动物中分离出来。为了解该菌的耐药表型及耐基因特征,本研究从中国西北地区575份羊肛门拭子样本中分离出一株产ESBLs广泛耐药(Extensively drug-resistant, XDR)的威斯康星米勒菌,命名为NX2019MW,采用琼脂平板稀释法进行最低抑菌浓度(Minimal inhibitory concentration, MIC)测定,并且对该菌进行全基因组测序。结果显示,该菌株对磺胺异噁唑、大观霉素、氨苄西林、四环素、氟苯尼考、头孢唑肟、头孢吡肟、头孢噻呋、头孢曲松、头孢克肟、博来霉素和恩诺沙星耐药,仅对美罗培南敏感。基因组分析表明,该菌株含有一条染色体和三个质粒,其中新型质粒pNX2019-MW1大小为219302 bp,且携带blaPER-4 和 blaOXA-10耐药基因。进一步分析表明,ISCR1可以介导全基因组测序(WGS)分析表明,NX2019MW的染色体长度为3437288 bp,含有三个环状质粒。其中,pNX2019-MW1属于新型质粒,长度为219 302 bp(GC含量为47.55%),携带aadB、catB8、blaOXA-10acc(6´)-IbqacEdeltablaPER-4sul1和 mph(E)等8个耐药基因。pNX2019-MW1(219 kb)是由质粒 p99101(CP028385.1)和pPROV013-PER(OK500122.1)同源重组而来形成新的质粒。blaPER-4的核心基因结构仍保留在ISEcp1-blaPER-4-gst的保守序列中,通过反向PCR发现,ISCR1可形成环状中间体介导blaPER-4水平传播。结合试验表明,携带ESBLs耐药基因的质粒pNX2019-MW1可在M. wisconsensis和受体菌株J53之间进行水平转移。因此,应加强对羊、人类和环境的中产ESBLs的M. wisconsensis监测,以防止耐药基因沿全链条进行传播。
